Add forms to your website


Once you have created a form you may want to add it to a website so that visitors can send you responses. When viewing the form tap "Embed form", this will open the following page that contains code used to add this form to a website.

While both will embed the form on your website, the JavaScript version is a little more sophisticated and will automatically adjust the height of the form as needed. However, some website creation tools will not allow you to use JavaScript; if this is the case then it is suggested that the IFrame version is used.

While the design of forms is consistent with the rest of UCare, this may look out of place on your website. If so, a custom CSS file can be specified to alter the design of the form, though you will need the help of someone with CSS skills to customize the design.


Here is an example prayer request form that has been added to a website. If this person's name and contact details match a person that you have on UCare, then when they submit the form it will be added to their profile. If, on the other hand those details don't match, then a new person will be added along with their form submission.
